How Citicoline Works in the Brain
Citicoline (also known as CDP-choline) is a naturally occurring compound that supports the brain’s structural and functional integrity. It is a precursor to phosphatidylcholine, a key component of neuronal membranes, and to acetylcholine, a neurotransmitter central to memory and learning.
Based on published research, citicoline appears to support brain health through several mechanisms:
- Neurotransmitter synthesis. Citicoline provides choline, a building block for acetylcholine, which plays a role in memory and attention.
- Membrane phospholipid synthesis. It supports the production of phosphatidylcholine, helping maintain neuronal membrane integrity.
- Mitochondrial function. Citicoline contributes to cellular energy metabolism in neurons.
- Antioxidant support. Studies suggest it helps mitigate oxidative stress in brain tissue.
- Neuroplasticity. Research indicates citicoline may support the synaptic adaptations that underlie focus and cognitive resilience.
There is some evidence from clinical studies in humans that citicoline may improve measures of cognitive function in older adults.
Clinical Evidence in Healthy Older Adults
Nakazaki et al. (2021) — Randomized Controlled Trial
Nakazaki et al. conducted a randomized, double-blind, placebo-controlled trial in 100 healthy adults aged 50 to 85 with age-associated memory changes. Participants received either citicoline 500 mg daily or placebo for 12 weeks. Citicoline supplementation was associated with significantly greater improvements in episodic memory and composite memory scores compared with placebo, as measured by computerized cognitive testing. This study was registered at ClinicalTrials.gov (NCT03369925) and provides direct evidence of citicoline’s effect on memory in healthy older adults.

European Food Safety Authority (2024)
The European Food Safety Authority (EFSA) reviewed the evidence for citicoline and memory function and identified the Nakazaki et al. trial as the key study in healthy older adults showing a beneficial effect of citicoline at 500 mg daily for 12 weeks on episodic memory. The review also noted that other randomized trials using higher doses or conducted in populations with dementia did not show similarly consistent benefit.

Fioravanti & Yanagi (2005) — Cochrane Review
The Cochrane review by Fioravanti and Yanagi included 14 randomized controlled trials of citicoline in elderly patients with chronic cerebral disorders, ranging from subjective memory complaints to mild cognitive impairment and dementia. The review found evidence of benefit for memory function and behavior, although the included studies were heterogeneous in design, dose, and outcome measures.

Świątkiewicz & Grieb (2023)
Świątkiewicz and Grieb, along with other reviewers, summarized that randomized controlled trials in cognitively normal middle-aged and older adults have reported positive effects of citicoline on memory function. At the same time, these reviews note variability in study quality, population selection, and measured outcomes.
